In this episode…

Many dentists struggle with maximizing their production while keeping overhead in check, often feeling stuck in a cycle of long hours and stagnant growth. But what if small, strategic shifts could double or even triple production without increasing work hours?

According to Dr. Spencer Greer, a highly successful practice owner and mentor, the key to exponential growth starts with mindset. He emphasizes that dentists must stop seeing expenses as costs and start viewing them as investments — whether in people, marketing, or systems. He highlights that by making intentional hiring decisions, optimizing case acceptance, and strategically structuring finances, dentists can unlock new levels of profitability. His own journey from an associate producing $60K per month to scaling two practices beyond $300K+ showcases the power of these principles in action.

In this episode of The Marketing 32 Show, Brett Allen and Dr. Spencer Greer, Founder of Big Time Dental Producers, talk about the mindset and strategies behind a high-performing dental practice. They explore why investing in the right people is the foundation of sustainable growth, how to maximize profitability while keeping overhead low, and the crucial role of case acceptance in boosting production. Dr. Greer also gives advice on balancing rapid practice expansion with personal and family priorities.

Quick shout out to Dr. Justin Waters and the episode we recently did together. Justin shares some insights and tips from turning his two office acquisitions that he did from. I think it was like $300,000-$400,000 in production to multi seven figures. And this was shortly after dental school. So super awesome episode. Go check that one out. Today though, we’ve got a new friend of mine who I met through Dr. Paul Goodman’s Dental Nachos Facebook group. Go join that group. There’s a ton of value there. He’s a highly successful practice owner, Dr. Spencer Greer of Big Time Dental Producers, where he helps dentists perform better and improve production. Spencer, welcome to the show. Oh, man. I was going to do it, but I didn’t. Spencer challenged me before the show to do it without saying welcome to the show. And I totally couldn’t do it. I couldn’t do it. I could not do it. Don’t. No, you’re not welcome on the show, Spencer. I’m taking it back.

Dr. Spencer Greer 01:56

Yes I am. You can still give me the feeling that I’m welcome without telling me that I’m welcome.

Brett Allen 02:01

Okay.

Dr. Spencer Greer 02:04

Well, Brett. Brett. I mean, the things that you were just saying about your guys’s guarantee for potential clients is a no brainer. I mean, everybody’s scared of paying money. Every dentist that I’ve met who’s a new owner, almost, I’d say 90% are scared of just paying money because they look at everything as an expense rather than an investment. And you know, the scariest thing about a marketing company is we have no idea what we’re going to get. Am I going to put five grand into something to where they’re just going to take it and automate me and just turn me into one of those little factory lines and just be like, all right, send them down the line. We don’t care.

Brett Allen 02:46

Yeah. And then and then get. Optimize their business for churn. Yeah, I’ve heard it, I’ve heard it.

Dr. Spencer Greer 02:52

We don’t know that. And so what you’re saying is, hey, if you put a dollar into my slot machine and you don’t get at least $2, then you don’t have to pay me. That just takes away all of that risk, all of that doubt. So you guys, you guys should be just anybody who just heard what you said should be super excited to give you a call and to give you your credit card, because that is not an expense. That is an investment.

Brett Allen 03:18

Absolutely. And marketing should be an investment. And I feel like there’s so many fly by night marketing companies out there running Facebook ads saying they’re going to get you more dental implants, saying they’re going to do your dental marketing, and how do we know if they’re actually going to deliver? So we do things differently here at Marketing 32. We try to vet who we work with beforehand. So we only want to work with practices we know based on some market demographics, based on some things that they’re doing internally with their practice management, how the patient experience is. So we’ve got some qualifications to work with us so that we know it’s going to be win-win. And so we’re not just taking anybody and everybody who’s willing to give us money. We really want to help dentists grow. So that’s our guarantee.

Dr. Spencer Greer 04:05

Yeah. Basically what you just told me in dental terms is I’m not going to do veneers on everybody who comes in and asks for veneers, like, yes, you are selective. You know what you’re good at. You know exactly who you can get results for. And you’re not just going to take everybody’s money. Yeah. So good for you. Good on you. That’s awesome.

Brett Allen 04:27

Well. Thank you. Yeah. So, well, let’s talk about you, Spencer. So I’m excited to have you on the show. I think the value you’re going to bring and the experience of the two practices that you’re owning, you’ve owned, and what you’re doing with big time dental producers. Excited to have our conversation today.

Dr. Spencer Greer 04:44

No, I appreciate you even taking an interest. Basically how your and my relationship grew as I became a sponsor on dental nachos, just like you. Which forced me to put myself out there more often if I’m paying money to be a sponsor, then I’ve got to take advantage of it, right? And so you’ve noticed that a little bit. The fascinating thing is it’s just so fascinating to where, like, I know the value of what I bring. I know the results that people are going to get. But the doubts are so thick, I don’t know. I don’t know who laid the groundwork to make it so hard to just get people to even want to inquire, to even want to listen.

Brett Allen 05:28

Yeah. You try to like self doubts or doubts from clients.

Dr. Spencer Greer 05:31

No, no. Just doubt. Doubt within the industry in order to get clients. Because. Gotcha. Once I get it, once I get it, once I get a client, it’s because I’ve told them I know exactly how to get you from A to B, and you want to be at B as soon as possible. Let me show you how. And then once they get there within weeks, I’m not talking months or years. Within weeks, once we get them to B, It’s just. It’s just like they tell me this was the easiest investment I’ve ever made. This was by far the biggest ROI. But the tricky thing, Brett is, is just people don’t know. People don’t trust quite yet. So the secret is, well, the secret that I’m trying to figure out is how can we get people to just see the vision of where we can get them, right? Yeah. I mean, I’m getting people producing on average, $40,000 a month within one month to $80,000. Wow. I’m getting people with three hours of my time where they are having a double of their offices, production, three hours of my time.

Brett Allen 06:44

No joke, can we go into that? Like can you share some of that? Like, what is it that you’re doing? Or what are you what are you looking at is like, is it staff? Is it marketing? What are you doing?

Dr. Spencer Greer 06:56

None of it is stuff. None of it is marketing, because those are not. Marketing is not my forte. I cannot speak to people on marketing. I can shuttle them to experts. But it’s important that we invest in marketing. Yes, staff, it’s all about investing in the right people. Man. I’m noticing a, you know, consistent series in which I keep saying investing, don’t I? But it’s just not being afraid of having these priorities in mind of you have to see staff as an investment. And if you have the right person, don’t be scared to pay them a little bit extra. Don’t be scared to fulfill your promises that you gave them when you first talked to them. Be like, all right, I’ve got to give her or him more than somebody else would. But it’s because I see this as fueling this rocket ship in my practice. So I went from being an associate producing the typical $60,000 a month. I say typical because that is the number one answer that I hear of what people call a quote. A good producing dentist is 60 grand. So, you know, I was producing anywhere between $60,000 and $90,000 a month. And then as I became an owner, I started producing $180,000. Granted, that was with me and one hygienist. Did you see that?

Brett Allen 08:26

I saw that, yeah. The balloon. You guys, you guys are watching on YouTube. The balloons pop up. I think it’s like a MacBook feature if you’re on or iPhone.

Dr. Spencer Greer 08:34

I just said. I produced $180,000. And then all these balloons just showed up on my screen.

Brett Allen 08:43

That I might be like a hand gesture or something. But anyway. That was that. Phenomenal. 62 so tripling. Yeah.

Dr. Spencer Greer 08:50

So but time out. That’s when I realized that it was just a simple shift of a couple of things that I needed to do. It was so simple. Fast forward. I fell into a situation where I owned two practices. I won’t go into how that happened. That’s a very long story. But at one point we produced 300 grand between the two. And I thought, oh my gosh, this is how can it get any better than this?

Brett Allen 09:20

That’s awesome.

Dr. Spencer Greer 09:22

Well, well, Brett. We have not increased the number of days or hours and we’re doing double that right now. So the thing is, is there is a trajectory to where there is constant growth possible. And we just have to keep putting our money and our energy into the right places. I definitely feel like, you know, you’re going to you asked me before the show, like, what was your greatest epiphany, you know, to have success in the practice. It’s just to understand that spending money is okay because you have to spend the money now. Don’t be stupid with it. Don’t be reckless with it. What you have to do is you have to be calculated. Now, I’m not counting every penny, but I am being careful about the pennies. You do a service of marketing to where somebody has to see this as something very valuable for their practice. People, dentists, owner dentists need to prioritize where they’re going to put their money. Don’t pinch every single penny everywhere. But really understand that. Marketing, staffing. What else? I mean, those are the big places where you should really put the dollars right. Especially good people. If you find somebody who’s good, do not pinch your pennies. Yeah. Do not pinch your pennies just for them to lose them, because your practice will suffer and your growth will deflate. You’re not going to grow to the way that you want. So I’ve moved out of clinical dentistry myself, and I have two doctors taking care of the production, one primarily at each office. And they both Brett. They both graduated in 1 in 2022 and 2 in 2023. They are new grads, totally handling $300,000 practices on four day work weeks. And that’s just them applying the principles that I teach people of just how to produce better. Now, is everybody going to produce huge numbers like these associates? No. But can they go from 60 to $160,000 a month? Yes. Can they go from 160 to 220? Yes. Like, absolutely. That’s what I focus on. So as I’ve moved out of clinical dentistry, I’ve kind of taken the opportunity to spread this to other doctors around the world. It was around the nation, but now we’ve gone to a few people internationally. So I get the right to say around the world.

Brett Allen 12:07

That’s awesome. Yeah. We crossed that bridge here recently. We’ve got some in Canada and Australia, so we’re international now.

Dr. Spencer Greer 12:16

Attaboy.

Brett Allen 12:18

That’s cool. So let’s dig into that. I know we talked about a handful of questions that we could go into, but you’re bringing up some really cool things here. How are you getting so investing in people, investing in. And, you know, you use this word investing. What is it that you’re putting money into? I know you’ve got programs and workshops, but can you give us kind of some hints as to like what’s what’s making this huge, outsized transformation and where you’re, where you’re making this kind of impact?

Dr. Spencer Greer 12:52

Well, it all starts with mindset. It all starts with mindset and just knowing that you have the potential. My overhead is extremely low.

Brett Allen 13:05

Okay.

Dr. Spencer Greer 13:06

I just partnered with a DSO that is 115 really high performing offices, and my profitability is number three out of those 115. So I’m not spending a lot of money.

Brett Allen 13:18

Wow.

Dr. Spencer Greer 13:19

Okay. So my overhead.

Brett Allen 13:21

Looking at. Expenses, reigning in overhead mindset.

Dr. Spencer Greer 13:25

Mainly it’s just being mindful of what your potential is to produce. Now here’s an example. Brett you’re a marketer. Do you know about how much a dental crown will cost around the nation? Have you been seeing what people would say of what a Crown range would be?

Brett Allen 13:45

That’s a good question. I’m not that hands on with it. My team does all that, but I think it’s like a thousand, something like that.

Dr. Spencer Greer 13:52

It’s all good. Just imagine, just imagine we say $1,500. Now you’ve got people who are people who have taken more C who do a high quality result, and they feel like they can charge like $2,000 to $2,500, but let’s just call the average $1,500. We’re going to have some listeners say that’s too high. That’s too low. I’m just throwing out a number. Now you do one crown with one build up. Let’s call that $1,800. What too many dentists are getting hung up over is a what the PPO will pay. So what? The insurance will pay for it. And they’re like, I don’t want to take that discount. Or B what the financing companies will chop off the top. And when they say, hey, give us 15% and we will fully finance this case for you. Now let’s take that example of $1,800 for the case. And let’s say it’s for crowns at $7,200 for the four crowns. What you’re saying is somebody can say yes to the $200 for four crowns today, but you’re kind of freaking out about the 15%, which is, let’s say, $1,000 off of that 7200. Man, I don’t want to give the financing company 15%. Would I say to that, Brett, is you’re getting $6,000 for doing a procedure that might take you two, 2.5 hours, three hours, I don’t now, six grand. But focusing on the top line. And just like, let’s go, let’s go. Would you rather be sitting on your phone and just. Yeah, and. Just waiting for somebody to come in just so that you can say to your friends, yeah, I charge fee for service fees. I don’t take discounts. My overhead is whatever. Like, sometimes we just got to take a look at those financing fees or the insurance write offs. If if it fits our model, it doesn’t fit for everybody, but it sure has fit for me. And with the overhead that I’ve enjoyed, it’s it’s a it’s a win. It’s a win. So that’s what I want to share with people is. That there are totally different methods of just going out and getting yours, and it doesn’t have to be where you’re getting full fee, cash price only. You’re not taking any write offs, any adjustments. Now, mind you, I don’t give patients big deals. I don’t let them. I don’t let them walk over me. I don’t let them get a two for one. That’s not what I’m talking about. I’m not talking about like charity work. But what I’m talking about is how to get people to say yes, how to get them to say yes, and how to get it now. Because if we say, let’s go, I get six grand for this production, boom. Then automatically we’re, we’re running, we’re getting them numb and we’re, we’re six grand extra in production on that same day production right there. So it’s pretty awesome. And too many people are focused on the wrong thing.

Brett Allen 16:54

Yeah. I love what you hinted at there. I think that’s a real big shift for the industry. I think you hear a lot about dentists who don’t want to be associated with discount dentistry. I don’t want to do all of this stuff where I’m paying under my fees. And you hear also a lot about people that are like, I’m switching out of insurance, I’m going fee for service. Tired of these guys putting the boot on my neck. But you’re saying, look, let’s find these profit centers and maximize the volume. Sounds like.

Dr. Spencer Greer 17:25

Yeah, I want to clarify that fee-for-service works for people.

Brett Allen 17:30

Yeah. That’s fine. Absolutely.

Dr. Spencer Greer 17:32

Yeah yeah yeah, that’s totally fine. I’m not. But fee for service. They also accept financing companies. And so they have to understand this.

Brett Allen 17:39

Yes this mindset.

Dr. Spencer Greer 17:41

Yes. I currently am mentoring somebody who went from PPO to out of network. And then all of a sudden he and his associates have really suffered in their production. And so as I’m mentoring him and his associate, he goes, oh man, we got to get back to network. So we get more patients. I said, absolutely, get a network. Like, don’t just be out of network so you can tell your colleagues that don’t this isn’t a status game. Pretend that we didn’t have social media and we didn’t have connection. How are you going to make money? How are you going to make dentistry profitable and work for you? So I, I don’t want people to just settle for all the lowest paying people. That’s not my message. Definitely negotiate the best fees that you can. But maybe, maybe in somebody’s journey of getting a lot of production, they’ll need to be in network for quite a while and it’ll work for them.

Brett Allen 18:41

Yeah. Yeah. So let’s go deeper on growth and kind of extrapolate some of the knowledge and wisdom that you’ve gained over, you know, managing two offices now working with a DSO. What’s been like the biggest epiphany or breakthrough that you’ve had that really has translated to like, it sounds like there’s many of them here, but going tripling, you know, that that income and, you know, making all this impact that you’re doing with your program. What epiphany have you had that’s been a real big breakthrough for you? Besides, you know, focusing on expenses and mindset.

Dr. Spencer Greer 19:21

Yeah. So when I asked my kids at the end of the day like, hey, what was the best part of your day? They usually start really thinking, oh, what was my best part of the day? And then I just say, you know what, just give me one good thing that happened, and then it comes a lot easier. So when you asked me what my biggest epiphany, when my biggest epiphany is, I’ll give you I’ll give you a really good one.

Brett Allen 19:41

Okay. Okay.

Dr. Spencer Greer 19:43

And that’s and. And that’s where I’ve realized that if I pay somebody to do something very important and it takes my duties off of that, then I can really focus on what I’m best at. So finding somebody who’s extremely good treatment planner, somebody who’s extremely good at closing cases to where we pass the baton to each other, where I say, hey, this person is going to take over the conversation here. She knows all the money stuff, I do all the teeth stuff. So I’m going to hand it over to you. And then that way she can hand it back over to me and be like, hey, let’s do it same day, go get them numb and we can have a nice little dynamic duo right there. So really finding somebody who complements you well and getting the case acceptance high and getting the treatment, treatment going and just wanting just wanting that success in the practice for you. And they’re going to take a higher investment for you. They’re going to take a higher wage, a higher bonus, whatnot. You have to be careful in how you judge that, but having somebody to take that from you can really increase the ability for you to just go crank, to go do a lot of good dentistry.

Brett Allen 20:52

Yeah. You mentioned so two things that are sticking out to me. Do you want a slice of a small pie or do you want a smaller slice of a really big pie. Right. It’s like that. That is a fundamental principle in entrepreneurship and business ownership. I, I recognize this with marketing 32 as well. We have phenomenal people on our team Francis, Sarah, Sean, Jeff, Lindsay. So I can name everybody because they’re all really awesome people. But yeah, trusting people to do a good job and then letting them run with it. So it’s the who, not the how, right.

Dr. Spencer Greer 21:31

And so for sure, you’ve. Got to have just good hearted people that you like and you want to win because you’re going to win side by side. And if you just have somebody who’s just filling a spot, you don’t, you don’t cheer for them to make a lot more money. You don’t cheer for them for their practice, to enhance their life and their lifestyle. You want somebody who you get in there and you’re like, I really want the best for this person, and. My practice is going to fuel that growth for both of us. So that’s a different. That’s a different mindset then rather than. I just want to practice that produces well with low overhead and I want to pay staff as low as possible. Like that’s what a lot of people just kind of think of and focus on.

Brett Allen 22:11

That’s awesome. I’m so glad you’re helping people make that shift. Cool. Well, let’s talk about a number of things that weren’t on our agenda here, but I think there’s a ton of value that we’ve unpacked. What’s been the hardest thing about like growing what’s been a challenge where it’s like, oh man, I’m I’ve got a couple in our business where you’re just like, banging your head against walls, like, man, what? I can’t crack this nut. What? So challenges and growing to practices challenges may be going to DSO. What can you share?

Dr. Spencer Greer 22:47

Yeah. What makes me really sad is when a husband and wife will go through dental school together, and the spouse is a great supporter through all the application and through the dental school and then associateship and moving multiple times, maybe residency, starting up a practice, maybe they even go through all those stresses together. And then comes a lot of success. And you see that marriage kind of breaking down. I really feel that a lot of doctors need to just keep family and marriage as such a high priority for themselves, even in the times of just exponential growth. Recognize that it’s not just you, recognize that it was your spouse who took care of the kids while you were starting your practice, or going in late night for those on-call emergencies. For me, I was married with two kids. We had four kids by the end of dental school, but two kids when we started dental school, and she was there to cook for me and to provide me emotional support and raise the kids. And so we’ve got to keep grounded that even if my name is on the practice, it’s me who people are talking about and talking to. Well, it was both of us. So one of the hardest things for me was just keeping that as a priority. And I really wanted to not let the practice keep taking all of my brainpower and all of my nights and weekends. It was just always on my mind. And I and I knew that my wife and my kids deserved way better than that. Life is not all about dentistry. Dentistry does not describe who I am or who we all are. It’s just a means to an end. It’s a means to an end. It’s a mode for us to pay the bills and to fund our lifestyle. And I really and I really hope that more people would take that into consideration of we go through this together as spouses. And to be mindful of how the family needs us, even during all those growth phases, because they’re hard and they can feel lonely. And our spouses deserve the best of us during all phases of growth, even even the end phases. So that’s my way of answering of what was the hardest thing is I just realized that dental business was taking over my mind way too much. Even off hours, even off hours. While I was at home, I would check my phone. I would reply to my staff. I would, you know, do payroll. I would just, just do too much stuff. So that’s why ultimately I just decided, you know what? I think partnering with the DSO would be really good so that I could. I’ve grown this. I’ve grown this thing to a really good spot. Let’s take some chips off the table and give a lot of the stress over to a support organization that can really handle it well for me. So that’s why I’m in the spot that I am. Plus, it gives me the opportunity to spend more time to coach others.

Brett Allen 26:13

That’s awesome. Yeah, I think I did. You hit on something big there. I mean, you’re your person you decide to commit your life to and you’re, you know, you’re in love with. Oftentimes we come home to them with, you know, we’re burnt out. We’re exhausted. We’re not our, we’re not the best version of ourselves for them. And why go through life like that without that awareness? So what I do something that’s helped me with this because it was a big challenge for a while was bringing work home and showing up. You know, deflated or, you know, you’re fighting all day, these battles, and then you bring that mentality home. You’re going to show up as a different dad. You’re going to show up as a different husband. And so something that’s helped me is a decompression exercise. At the end of my day, I kind of unload, I do a little bit of breathing exercise. I start to focus on who do I want to be when I walk through that door. And I make a conscious effort to do that every day. So I have an alarm that goes off on my iPhone that I take 15 minutes, and I just kind of focus on what’s important to me. I focus on the vision I want for my life and my family, and that helps me center. It helps me kind of come back to, okay, I’m checking out of work mode, I’m checking out of Hunter mode, and I’m going into dad and husband mode. So that’s something that’s helpful. But well, let’s let’s wrap this up I know or. Yeah. Go ahead.

Dr. Spencer Greer 27:45

Just my, just my last thing about that. And about five years ago I was in a hotel room with Steve Dove. He is a doctor. Dove is out of Missouri, and when I met him, I’m like, are we, like, best friends? You know, it was just such a cool connection. So we decided to share a hotel room together and just. I remember just chatting that night, we were each in our own beds and he’s just like, yeah, I take a pre-workout in the morning and a pre-workout at the end of my work day, and it’s just like, why do you do that? He’s like, oh man, it gives me just all the energy, all the positivity that I know that my kids need. I just like, oh, that is such a fantastic thought, you know. Because, yeah. So many times our 8:00 patients get the best version of ourselves. And then, then at the end of the day, we just go home and it’s just like, oh, they have to get the scraps from us.
