- [21:52] The critical importance of assigning clear team roles in the aligner conversion process
In this episode…
Most dentists know clear aligners can be a major revenue driver, but actually getting patients to say “yes” is a different story. Practices often rely on marketing to bring in leads, only to lose them due to shaky internal workflows and unclear roles. What if the key to scaling aligners isn’t more ads, but better in-office conversations?
According to Dr. Avi Patel, a leading voice in aligner implementation for general dentists, the answer lies in converting the patients already in your chair. He highlights that most dentists don’t need more leads — they need better systems for communication, team handoffs, and patient education. By focusing on predictable cases and teaching every team member their role, practices can dramatically boost case acceptance without external marketing. This shift not only increases production, but also improves patient outcomes and practice morale.
In this episode of The Marketing 32 Show, host Brett Allen sits down with Dr. Avi Patel, Founder of Clear Aligner Advisor, to talk about how dentists can generate millions in aligner revenue without flashy marketing. They dive into internal conversion workflows, team-driven patient education, and why pitching oral health over cosmetics makes all the difference.

Quotable Moments
- “I was approaching it from oral health, not from cosmetics — and that was the key.”
- “This is what’s going to happen. These are the big scary monsters that you’re going to have to face.”
- “Your desire to want to know every single one of those steps is actually what’s holding you back.”
- “Has anyone ever talked to you about improving the position of your teeth for your oral health?”
- “This is a big decision and it’s a big investment. We understand that.”
Action Steps
- Assign clear roles in the aligner workflow: Defining responsibilities for each team member creates consistency and improves patient conversion rates.
- Talk to every qualified patient about aligners: Initiating these conversations uncovers hidden opportunities and normalizes aligners as a standard treatment option.
- Focus on oral health benefits over cosmetics: Shifting the narrative builds trust and helps patients understand the medical value of aligner treatment.
- Use micro-commitments to reduce decision friction: Small upfront deposits ease financial hesitation and increase case acceptance without overwhelming the patient.
- Invest in team training, not just marketing: Equipping staff with scripts and confidence enhances results from existing leads and maximizes internal efficiency.

Go to dentalscheduletemplate.com Check out the episode. It’s awesome today! Super excited to have Dr. Avi Patel on the show. Dr. Patel is a dentist and Founder of Clear Aligner Advisor, dedicated to helping general dentists confidently integrate clear aligners into their practices.
He’s recognized a significant gap in aligner education for general dentists wanting to offer this transformative procedure and create, and he created a comprehensive platform to bridge that gap. We’re going to talk all about that today. So Dr. Avi, he’s supported over 200 dentists who have collectively generated more than $9 million in aligner revenue. Very impressive. Super excited to have you on the show today, Dr. Patel.
Dr. Avi Patel: 01:54
Yeah, pumped to be here. Thanks for having me. Also shout out to Kirk. He is awesome. Anybody that didn’t listen to that episode.
I didn’t even listen to the episode, but I already know that it is probably fire because Kirk is a genius. Him and his wife really do have something very special there. But yeah, man, thanks for having me.
Brett Allen: 02:13
Yeah, I’m excited to dig deep on your story and learn how you’re doing niching down and focusing on aligners. It’s a huge revenue opportunity, like you said. So let’s kind of, I guess, to bring everybody to present. Let’s go back and you know, how out of dental school you started as an associate. Talk to us about that journey.
Dr. Avi Patel: 02:35
Yeah, yeah. So I like to just kind of start even at the beginning of, like, why did I want to be a dentist? And so growing up, I think I, I mean, not even I think I was, I was five, ten in like middle school. So I was one of the tallest kids. So I thought I was destined for the NBA.
No question about it. And then yeah, high school came around and went to being one of the tallest to like the shortest or average. And then that’s when I realized, like, all right, NBA probably isn’t going to work out here. So then growing up in Indian household, my parents were very much pro, you know, doctor, dentist, lawyer. So I was like, well, let me just see what this, what this is all about.
And at the time, my uncle was a dentist up in New York. So I just went to go shadow him for a summer, went to go kind of see, you know, him in his practice and his element and I and I was drawn to honestly his lifestyle. Like I kind of I was like 16 at the time and I’m like, dang, this guy’s got a really, really cool life. Like, he’s got a nice house, he’s got kids. His vacations throughout the year, works a couple days a week, golfs.
Always in a good mood. So I’m like, you know, I think I want something like that. Watched him in practice and I kind of saw the next level, like patients coming in pain, them leaving feeling better and just kind of just the overall vibe. And so I was really attracted to that. And then he had a dinner party when I was there, and he was having a lot of fun with everybody.
And then one of his friends was talking to me, and he’s a neurosurgeon. He’s and he’s kind of, you know, talking to me. I’m 16 and he’s just like, hey, so, you know, sounds like for your career, like you want to, you know, I think at that point I wanted to like, I was considering, like, sports medicine or dentistry. He’s like, well, you have to make a decision. Like you got to choose med school or dental school.
So I’m like, yeah, you’re right. And so we’re like diving into it. And then I’m just like, by the way, why are you like talking to me? Like, why are you spending so much time with me? Why aren’t you hanging out with them?
And he’s just like, oh, like I’m. I’m on call. And he was like a Friday night, and I was just like, oh, that sounds horrible. Like, like, I don’t want, like, if I have to work that hard to to like at that point, that’s where my mind was. So I like to share that story because it kind of lays the foundation of how I view dentistry and where my priorities were at.
Like I was drawn to dentistry. I think for the whole concept of time, money and fulfillment. Right. And the person who embodied that was my uncle. So that’s what drew me in.
I think that’s what draws in most dentists, regardless of what they put on their dental school application and personal statement. It’s, you know, it’s a combination of those three. And I think that’s the cool part. And then yeah, dental school was I mean it was a grind. But I also I had a lot of fun kind of found the happy medium between getting, you know, like passing essentially, but also like understanding concepts but then enjoying time outside of school.
I went to NYU, so I was in Manhattan. I was 21 years old.
Brett Allen: 05:34
Oh, I bet that was fun.
Dr. Avi Patel: 05:35
It was a blast. It was a blast. I had student loans, so, like, money’s not real, you know, for those four years, because, you know, you’re going to make millions of dollars when you’re a dentist. That’s what they tell you. Right.
So it was awesome. And then I come out and it was the complete opposite. It was like the worst. I was working six days a week. I was trying to figure out and learn how to be a dentist still, because that’s just how it is.
You know, I was looking for a mentor. Everybody said, find a mentor. Find a mentor. You’ll be set. And the reality was, is it was tough.
Like, my uncle was a resource, but like, he was a business owner. So like, he had a practice to run. And he and his practices, he didn’t necessarily have capacity to bring me on. So, like, I didn’t get that luxury of just walking into a practice, you know, waiting for me like, like some dentists do. And so it was a challenge.
But I looked at every opportunity, as, you know, a learning experience and practice in like eight different offices my first two years. So there was a lot of perspective — a lot.
Brett Allen: 06:43
Yeah. Yeah.
Dr. Avi Patel: 06:44
So I saw a lot and then and I got to grow. I did get to grow because I understood more than just clinical. I got to see, you know, team dynamics like workflows, onboarding, scheduling, like all this kind of stuff. And so that was all helpful and useful. You fast forward then I got to the point where I was I burnt out like I was doing bread and butter, but I burnt out on it and I was like, man, this sucks.
Like, this is not something that I can see myself doing forever. There’s got to be something more here in this profession. And so then that’s when I decided that I was going to learn how to do implants and aligners. And at the time, my wife and I moved from New York down to Texas in Austin and worked for a DSO. And they basically provided both.
They were like, hey, you can hey, you know, we’ve got this continuum with implants. You can learn implants, you know, and then we’ll set you up with like this webinar series for aligners. And I kind of went down both, but I caught a lot more traction with the liners. I had a mentor doctor blocker leaned on her pretty much was like, hey, what do I need to know? What do I need to refer?
So that way I can just provide, you know, entry level care to my patients with the liners and that I don’t get stuck or in trouble. And she helped me out. She helped me review cases. I went on to go implement everything myself. Started 50 cases in my first six months.
I didn’t I didn’t think that was like impressive. I just thought that was like, what you do, like you just talk to patients about aligners and then they start. So I was very naive. And then the aligner reps just started coming in. They’re just like, hey, what are you doing?
And I was like, oh, I’m just talking to my patients. Like letting them know what’s going on and how we can help. And they were saying, yes. And this was in a blue-collar practice. So state employee, I think the practice, the practice may have been older than me, honestly.
So it was not like a modern boutique office.
Brett Allen: 08:57
Okay.
Dr. Avi Patel: 08:59
But I was approaching it from oral health, not from cosmetics. And that was the key. And so for me, at that point, I recognized that, okay, I guess I am doing something differently because these, these, these reps are really pouring into me. And so that’s when the next kind of lightbulb moment happened where I was, I went from, you know, and I know some people get weird about talking about production and stuff, but I don’t I think it’s important because if you’re not producing, you’re not helping people and you’re also not making money. So.
So yeah. So I went I walked into this group and I was probably I mean, like any new doctor, I wasn’t one of the highest producers in the first month. But my goal was to be a top producer. And then within like three months, that’s what I did. And the doctor I was working with, he had like, I mean, he had about 20 some years of experience and he was just like, hey, I don’t understand, like you’re producing.
Yeah, you’re producing like a hundred k a month and you’re barely on the clinic floor. Like you’re just sitting behind your computer. And I was like, oh yeah. Like I, I mean, I got 50 K of production this month just from doing aligners. And then yeah, I did some crowns and some fillings.
And he was just like, can you teach me? And I’m like, yeah, I’ll teach you. So then I started teaching him case selection conversations, all that. He started to have success.
Brett Allen: 10:32
That’s awesome.
Dr. Avi Patel: 10:33
Yeah. So I was like, this is. So then he started to have success. And then I was like, well, this is pretty cool. Like, it was just fun.
Like, now we’re on the same team, right? We’re helping our patients producing. And so that’s when the next.
Brett Allen: 10:45
Marketing is just purely leveraging patients in the chair having a conversation. That’s it. You said you focus on the the That. Not the cosmetic, but the dental benefit.
Dr. Avi Patel: 11:00
Correct.
Brett Allen: 11:00
And so you go deeper on that like.
Dr. Avi Patel: 11:03
Yeah. No, we weren’t treating every patient, but we were talking to every patient that could qualify for it. So this is where like I’m a fan of referring to your specialist but simple predictable cases. You can do so. I mean, the list of what you should refer essentially is, you know, bilateral posterior crossbites class three cases.
I wasn’t treating teenagers. So, you know, referring teenagers, deep bites greater than four millimeters, cases like that are really just more extensive and require. Right. Honestly, just more a broader scope. I believe that you should refer that to ortho.
So I was keeping everything, you know, everything that was pretty simple on my plate. And so that allowed.
Brett Allen: 11:51
Pitching everybody that came in. Hey, you should. Have you thought about doing a-liners? Just kind of like, open the door more. Not even if they.
Yeah. If you saw a need, like, hey, you’ve got some crowding happening here or. Yeah.
Dr. Avi Patel: 12:07
Yep. Just highlighting it, just being like, hey, you know, your teeth aren’t in a healthy position. Has anyone ever talked to you about improving the position of your teeth for your oral health? And they would be like, no, no one’s ever talked to me about that. I’m like, would you like to hear a way that we can help?
And they’re just like most of them at that point if they say no. Cool. End of conversation. Right. But if they say, yeah, I at least get to go to the next step and talk to them about clear liners.
So cool.
Brett Allen: 12:37
Yeah. Cool. So you’re so you’re in this DSO. You’re starting to figure out I’m on to something here.
What happened next?
Dr. Avi Patel: 12:46
Yeah. So now I’m like, man, I want to I think I want to do more of this. Like, it’d be kind of cool to, like, maybe cut down on a clinical day or two and just do more teaching. And so I proposed to the group, the DSO’s, like, hey, maybe we can make a role or something for me and I can teach the rest of the offices. I mean, they saw the value in it, but at the time they were just like, no, we want you to produce.
So I was like, okay, dang. So my wife, she’s in finance, so non dental. And she was just like, why don’t you just consult? And I was just like, I don’t even know what that means. And so consulting is basically where you get paid to solve or help solve some advice.
Yeah. And so I’m like, all right I can do that. I mean, I was literally talking to friends, teaching them on the phone, on my drives home.
Brett Allen: 13:36
Colleagues, everything. Yeah.
Dr. Avi Patel: 13:38
How passionate I was about it. And so she’s just like, you need to get paid for this because you are making them hundreds of thousands of dollars. And I’m like, yep. Yeah. I guess, like I could do that.
So then I started doing consulting locally, one on one with doctors in Austin. They would get results. So one of the doctors I worked with, new practice owner, acquired a practice. He wanted to learn how to integrate and do aligners, and he started like 25 cases in his first two months. And so no marketing, right?
Brett Allen: 14:13
And yeah that’s awesome.
Dr. Avi Patel: 14:15
And so then from there I kind of had now this like curriculum. And so my next thought was like, all right, I want to get more doctors that I can help. So then I turn to social media and then I just started making content. And so I post Instagram and YouTube videos as an organic way to market. Again, I don’t — I knew nothing about marketing.
I knew nothing about content creation, but I knew that I did not have the money to invest in full blown marketing. So I was like, let me try. Let me try like.
Brett Allen: 14:48
Throw things at the wall, see what sticks.
Dr. Avi Patel: 14:50
Yeah, yeah. I was like, you know what? It never. Everyone says social media is the way to go, so let’s give it a shot. And so that’s what I spent a lot of time doing and I got a lot of interest.
But this one on one model was not scalable. And it was tough because there would be doctors from other countries reaching out. And so there was just a bunch of problems there. Logistically. And then so then the next mentor in my life came up, Dr. Rinesh Ganatra.
And he was like, you should make an online course because he’s like, it’s scalable, you can actually help more people. And he’s just like, it’s just it’s just a better use of your time. So I’m like, okay. So he kind of guided me through that. I looked that up.
We launched a course, did a webinar, promoted that, and we got like four doctors to sign up on the first webinar that we did. And I was just like, oh my God, this is crazy. It took the doctors through the program. They came in on the other end. They had the confidence.
They were, you know, they were doing a-liners. And that’s when I was like, all right, I think I need to do more of this. And so shifted into that, started just doubling down on content as a way to just essentially the whole vision with this thing is to make a liner education accessible to as many doctors as possible. And anything that I work on now kind of goes through that lens. So whether it is free content, paid courses, paid support, it is all through that lens of like, is this going to help make education more accessible?
And so yeah, it’s been a lot of fun focused on that. Got to the point where it was very clear to me where like the pool, I call it, the pool of the universe was taking me in this direction to go deeper with this. So then I went full time with open.
Brett Allen: 16:47
Gotta walk through it. Yeah.
Dr. Avi Patel: 16:48
Went full time with it. End of 2023. And, you know, like with anything, any entrepreneur business owner knows that the initial stages are always challenging. And so when I went full time with it, we had a lot of challenges. It was, you know, I.
Brett Allen: 17:07
Heard Alex Hormozi, I heard him say uninformed optimism. You go through this stage of, oh, this is exciting opportunity, and then you’re in it and you’re like, oh, here’s all the things that I’ve got to figure out.
Dr. Avi Patel: 17:20
Yep, that was exactly where I was at. I love Alex Hormozi, by the way. Huge fan.
Brett Allen: 17:26
Yeah.
Dr. Avi Patel: 17:27
And I think every dentist needs to go to his workshop. I went and it is phenomenal.
Brett Allen: 17:32
Oh yeah. Awesome.
Dr. Avi Patel: 17:34
Yeah, I went last year.
Brett Allen: 17:35
We’ll have to talk more about that.
Dr. Avi Patel: 17:37
Yeah, we definitely can. But yeah uninformed optimism. And then I was at that point where like, you know, enrollments weren’t coming in. There was just a bunch of challenges. And so I was just like, all right, what do I do?
I could have just stopped. I could have just went back to practicing. I could have just given up. But it was never an option for me, because I knew that I needed to figure this out and stick with it, because nobody else is working on this.
Brett Allen: 18:06
Like this. With marketing agencies out there that target specifically docs to help them grow aligners. But I don’t think they’re doing it how you’re doing it. They’re just generating the leads and passing it off to the doc. And we talked about this before the show.
That will expose a lot of holes in your process and problems. If you turn on marketing and you don’t have a clear patient experience, how do you pitch the patient? How do you review the case? Finances. All of that.
How do you get the yes once they’re in the practice? So that’s a lot of what you focus on right with your program.
Dr. Avi Patel: 18:43
That’s it. Yep. And then what we do is we make it approachable. Right. Because I you know, I am very close to the beginning, right? I mean, I’m not like there are other educators out there that exist, and I do think they all bring an extremely high level of value. But like with anything, everybody has a different style. Everyone has a different perspective. Everyone has different philosophies.
And I’m not saying mine is, you know, the only one or the right one. I’m just saying I’m probably the one out there that’s been closest to the start out of anybody else. And so I know the challenges. I know the thoughts that keep you up at night. I know the you know what may be a small bump to a, you know, an educator who’s been in the game longer is actually a mountain to somebody starting off right.
It’s the first mountain. It’s like, how on earth am I going to talk to this team member to help me? And like that stuff that I think a lot of people just skim over. They’re just like, yeah, just tell your team to scan, make them scan, and then you just talk about it and go and it’s like, yeah, like obviously you want them to scan, but like, how do you have that conversation? How do you —
Brett Allen: 19:55
Yeah.
Dr. Avi Patel: 19:56
How do you do it in a way that is not bossing them around? That is going to then just add to the toxicity of the practice. How do you share your vision? Share your goals. Empower them.
Give them their role. Right. So that way they actually feel connected to the practice and to this mission of elevating the standard of care. So this is all stuff that I go over in, in our programs. And yeah, I think it’s, I think it’s, it’s kind of more of a way of like, you know, I’ve been I’ve been in your shoes not that long ago.
Brett Allen: 20:31
Yeah. Yeah.
Dr. Avi Patel: 20:33
This is what’s going to happen. These are the big scary monsters that you’re going to have to face. But this is why you don’t need to be afraid of the monsters. Like, here are the tools that you need. And you can go ahead and you know, and take those steps forward without being scared.
And so yeah, I mean, it’s a lot of it’s.
Brett Allen: 20:52
Let’s dig a little bit on that. So like we’ve worked with a lot of practices. We’ve helped over 240 practices grow at this point. And there’s been a lot that want Invisalign help. So we can certainly dial in a marketing campaign to help generate those leads.
We had one doc in particular we generated. We sent out one text message campaign. They got 24 accepted Invisalign cases from it. And I was like, whoa, what? What are you doing?
And it was all things internal to the practice. It was all how they talk to the patient. What’s the process like when you go from chair to get the scan done? What financing options. All of that.
They have this stuff dialed in. And so if you could just share one thing with us about like internal dynamics, like what that conversion process is like, what do you think is like the biggest thing that you see most docs struggling with when it comes to getting patients to say yes to a case?
Dr. Avi Patel: 21:52
Yeah, I think it’s they don’t have the script or the workflow of everybody’s job in the process. This is not just a doctor. Say this script and magically you’ll start cases. Know everybody on the team needs to know what their role is right from the second the patient walks into the door. Who is grabbing them?
Where are you seeing them? Who is scanning them? Who is reviewing the scan with them? What are you saying about the scan to them to seed the problem that they have? And then how do you make that transfer to the next person, which is usually the doctor.
Right.
Brett Allen: 22:33
So yeah.
Dr. Avi Patel: 22:35
And then from the doctor, how do you then highlight those points that is in alignment with what your team member just said. And then take that and then offer the solution of aligners, navigating this conversation with the patient, answering any objections that come up. So that way, by the time you’re done with that conversation and it moves to financing, the first of the two decisions is done right. The first decision the patient needs to make is, is this something that I need to do? Yes or no?
They need to decide that. Do they need it or not? The dentist job is to make that help them make that decision for them. That is it. After that decision is made that I need to do this, the next decision is how am I going to pay for this?
And that is what the treatment coordinator does, right. And so then there’s a handoff from the dentist to the treatment coordinator, mainly because most dentists are afraid to talk about money. That’s a different conversation. So then that handoff now goes to the treatment.
Brett Allen: 23:32
Doesn’t have to be though, right. It’s like typically you see a treatment coordinator aka sales person. Yeah. Yeah.
Dr. Avi Patel: 23:41
I mean, some of the top dentists are doing it themselves and it’s just it all comes down.
Brett Allen: 23:46
To right when you started out, you were just talking pitching it. Yeah.
Dr. Avi Patel: 23:51
Yep. You have to. And I think patients like you’re in the moment, they want to know, like you don’t need to hide from the money conversation. It’s just that’s the next level. The second somebody asks how much does it cost?
They’re 90%. They’re like 90%. They’re right. It’s your job to now just make it so that that financing conversation.
Brett Allen: 24:10
Isn’t a barrier.
Dr. Avi Patel: 24:11
Yeah, exactly. And so that’s where the second part of that process comes in, which is they need to decide how they’re going to pay for it. And so what we teach doctors is helping your patients separate these two experiences. So what I mean by that is when they’ve already decided that they need it because they’re answering all the clinical questions, the problem, all that is highlighted and how this can benefit them. Now when they get to the coordinator, it’s like, hey, so here are the different ways that you can pay for this treatment, right?
Whether it’s in-house, third-party, pay-in-full, all that. This is a big decision and it’s a big investment. We understand that. So to make this easier for you, so you have the time to actually think about and talk about this, all we ask is for a small deposit of $250. And then when or sorry.
Yeah, small deposit of $250. And then you have the next 4 to 5 weeks to talk to who you need to, you know, move money around as you see fit and budget accordingly so that when the trades are in you then tell us at that appointment which option you’d like to proceed with. But this way we can start moving forward, and then we can get you the treatment that you need. Because we’ve already agreed and discussed that this is something that you decided that you need. And so it’s like, right.
And now, now, now. And to call this the easy start method because you just that’s all you need. You just need that commitment. And what that does from a psychological standpoint is now this patient is because most patients. Right?
Everyone needs to talk about it with somebody. Everybody needs to see that. Cool. Which is fine. Like, I get it.
I don’t know, the last time I paid for something $5,000 outright. Like I also sometimes I just need to talk to my wife and be like, hey, like, this is something we should do. I’m about to get this, like, nose procedure done for like my turbinates to help with, like my breathing and like, yeah, I was just like, hey, let me just talk to my wife about it. And they were like, cool, go for it. So that being said, though, you do this now when they go home and talk to their spouse, it’s, hey babe, I need this procedure.
This is why, how can we pay for this? How can we make this work? Right now you’re trying to solve for the how. Now you’re troubleshooting. Now you’re actually thinking for a solution.
You’re not asking permission. Like you don’t need like asking somebody else about a decision that’s going to impact your own health, which is then going to lead to more expensive bills in the future. Because if you don’t get your teeth in alignment, you’re going to have right. You’re going to have more problems.
Brett Allen: 26:57
Problems, problems.
Dr. Avi Patel: 26:58
Yeah, exactly. So it’s going to be expensive. So and like at the end of the day, like your spouse is not the doctor like your spouse is not the person that diagnosed you. So the person that’s the actual.
Brett Allen: 27:10
Expert seeing a smile that’s straight. So yeah, there’s a lot you can do there to help frame the discussion. But I love the tactic of, hey, let’s just, you know, let’s have a micro-commitment here. Let’s get this thing started. Finances will take care of itself later, you know, we’ll figure it out.
Dr. Avi Patel: 27:30
Right.
Brett Allen: 27:30
Yeah. So this is great. Well you guys, as you can tell, Avi’s got all the things figured out. So you don’t help.

One more thing before we go. I always ask if you could have. If there’s one epiphany, you could help a doctor to have that one domino. If you could help them, just push that over and make a huge impact for their practice. In your experience, what’s that?
What’s that one thing that you could just tell them right now?
Dr. Avi Patel: 29:00
Yeah. I think your desire to know every single step in a, in a, in a multi step process, your desire to want to know every single one of those steps is actually what’s holding you back from starting.
Brett Allen: 29:15
Yeah. Yeah. What do they call it? Analysis paralysis. Right.
It’s like you can’t, you got to kind of jump in, take what knowledge you have and execute and you learn and pick up the pieces as you go. Build the parachute on the way down. Right.
Dr. Avi Patel: 29:29
And what and what it is. Yeah. And it’s it’s it’s take the be willing to take action and take the first step with someone you can trust. Right. And I think that’s the key.
And with anything like anything, anytime you do something for the first time, you don’t really know what you’re doing, and you’re not going to know until you do it. And yeah, if you have someone that you trust that has your back, take the first step.
Brett Allen: 29:53
